What is TMJ and how a chiropractor can help

TMJ pain — that ache, click, or sharp catch in your jaw when you eat, talk, or wake up — is rarely just a jaw problem. In most cases, it traces back to how your head sits on your neck. This post explains what TMJ is, why a chiropractor is often the right person to evaluate it, and what treatment at our South Loop office looks like from your first visit through correction.

What TMJ actually means

TMJ stands for the temporomandibular joint — the joint where your jawbone (mandible) meets the temporal bone of your skull. You can feel it: trace the corner of your jaw upward toward the bony arch just under your cheekbone. That’s the hinge.

The condition we call “TMJ” is pain and dysfunction in that joint. Classic signs are jaw pain when chewing, talking, or first thing in the morning. The pain itself varies — dull and achy for some people, sharp and stabbing for others. It can also refer into the face or to the back of the skull, which is miserable.

The joint is driven by large muscles that control how your jaw moves. When the jaw or the neck above it shifts out of alignment, those muscles overwork, adhesions form in the tissue, and the joint surfaces wear unevenly. That’s where the pain comes from.

Why see a chiropractor for jaw pain

Chiropractors train extensively on how joints should move and how the nervous system controls them. Your skeleton has two parts: the axial spine (vertebrae, sacrum, pelvis) and the appendicular skeleton — the joints that anchor into the spine, including shoulders, hips, knees, and the jaw.

Research coming out of the American Dental Association — and we work with a number of dentists here in Chicago — points clearly to the cervical spine as a driver of TMJ pain. The short version: when your neck loses its natural curve, your head shifts forward, and that shift drags the jaw out of position. Use a misaligned jaw thousands of times a day to talk and chew, and you get pain.

What a healthy neck looks like

If you stand sideways in front of a mirror, your ears should sit directly over your shoulders — top bone on top of the bottom bone. From the side, the cervical spine should hold roughly a negative 42-degree curve. Trauma (car accidents are a big one) and repetitive stress (desk work, phone use) flatten that curve over time. As it flattens, the head drifts forward and the jaw follows.

How we treat TMJ at Advanced Health

Our approach is a three- to four-pronged attack.

1. Restore joint function with adjustments

Every joint in the body should move freely and stably. The top two bones of the neck — C1 and C2 — carry your skull, and the nerves exiting that area feed the eyes, ears, nose, and throat. When a TMJ patient is subluxated at the upper cervical level (meaning the joint has shifted or isn’t moving well), it creates stress on the joint, soft tissue, and nerves right where the jaw is hinging. Adjustments get those segments moving again.

2. Correct structural shifts with spinal traction

Adjustments alone won’t change the shape of your spine. To restore the cervical curve and pull the head back over the shoulders, we use spinal traction protocols based on Chiropractic BioPhysics. 3. Release the soft tissue

When muscles are chronically overworked, they develop adhesions — think of glue inside the tissue. The masseter and especially the pterygoid muscles are major culprits in TMJ pain. We use targeted soft tissue therapy on those muscles to break down adhesions and restore healthy glide.

4. Rebalance with postural rehab

Forward head posture comes with what’s called upper-cross syndrome: some muscles are doing too much work, others not enough. We use physical therapy and corrective exercises to retrain that balance so the correction holds.

What your first visit looks like

Your first visit is about evaluation, not treatment. We don’t guess.

  • Consultation. You’ll fill out an intake form, and one of our doctors will sit down with you to walk through your history — when the pain started, how often it shows up, how it’s affecting your life.
  • Orthopedic exam. We take your spine through its range of motion and check how each joint is moving.
  • Neurological exam. A series of tests to see whether the nervous system is involved.
  • X-rays. Chiropractors read X-rays a little differently than medical doctors. Yes, we screen for pathology — fractures, infections, anything unusual — but we also evaluate biomechanically: how is your spine different from normal, and how is that shift creating tension on the joint, tissue, or nerve that’s driving your jaw pain?

You’ll go home after that. We use the time between visits to study your case carefully.

On your second visit (the report of findings), we walk you through everything we found, explain what’s causing your pain, and lay out the treatment plan that fits your case. Then we get to work — joint function, spinal traction, soft tissue, and corrective rehab, in the proportions your case needs.
